This position involves comprehensive patient care while maintaining clear and compassionate communication with patients and their families. The LPN will educate patients on health maintenance and treatment plans, promoting recovery and enhancing quality of life. Working collaboratively with healthcare teams, the LPN ensures compliance with nursing standards and regulatory requirements. Ultimately, this role is essential in providing personalized, attentive care that supports the physical and emotional needs of each resident in a safe and supportive environment.Minimum Qualifications:

  • Valid and current Licensed Practical Nurse (LPN) license in the state of practice.
  • Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare team members.

Preferred Qualifications:

  • Experience with electronic health record (EHR) systems.
  • Strong interpersonal skills with a focus on patient education and advocacy.
  • CPR and First Aid certification.

Responsibilities:

  • Provide direct patient care including monitoring vital signs, and administering medications.
  • Educate patients and their families on health conditions and treatment plans.
  • Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans.
  • Maintain accurate and detailed patient records in compliance with healthcare regulations and facility policies.
  • Respond promptly to patient needs and emergencies, ensuring patient safety and comfort at all times.
