Your Role and Responsibilities
- Conduct assessments for patients in both inpatient and outpatient environments, designing tailored treatment plans
- Implement physical therapy techniques centered on mobility, strength, balance, and functional performance
- Collaborate with a diverse team of nurses, physicians, case managers, and rehab specialists to maximize patient outcomes
- Guide patients and their families through therapeutic objectives, discharge planning, and home exercise programs
- Ensure thorough documentation of evaluations, treatments, and patient progress in adherence to facility and regulatory standards
- Engage in care team evaluations and interdisciplinary discussions to promote cohesive care strategies
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
- State licensure as a Physical Therapist
- CPR certification preferred
- Additional certifications (e.g., neurological, geriatric) welcome, but not mandatory
