Ebenezer Healthcare Access is hiring Community Health Workers to provide care coordination services to the immigrant population located in both Columbus and Cincinnati. Community Health Workers will support our Telehealth station, promote the healthcare referral system, and home visiting program in partnership with local healthcare organizations. Prospective candidates should be knowledgeable about current community programs and resources. The position requires the ability to assist individuals in overcoming cultural and language barriers to access healthcare and community resources. This job is located in the Dayton/Columbus area with a negotiable salary.

Job Summary

This position is responsible for supporting immigrants in navigating healthcare and community resources. The Community Health Worker will assist clients in accessing medical care, connecting with local programs, and overcoming cultural and language barriers. The role also involves community outreach, support for telehealth services, and care coordination in partnership with local health organizations. This part-time position offers a meaningful opportunity to improve health outcomes for underserved populations.

Responsibilities

  • Maintain accurate client records, including notes, forms, and supplementary information; perform weekly updates for efficiency.
  • Meet with clients and community organizations to design and implement health programs for individuals and groups.
  • Connect immigrants to healthcare providers and assist with scheduling medical appointments.
  • Conduct community outreach via phone, in-person visits, and other media to provide guidance and personalized recommendations.
  • Complete administrative tasks to ensure proper documentation and reporting.
  • Perform other duties as assigned by the management team.

Required Skills/Abilities:

  • Strong interpersonal and communication skills to build trust with diverse populations.
  • Ability to navigate and explain healthcare systems and community resources.
  • Proficiency in care coordination software and Microsoft Office (Word, Excel, PowerPoint).
  • Multilingual abilities, including French, English, Kinyarwanda, Kirundi, Kinyamulenge, and Swahili; Spanish or Arabic is a plus.
  • Knowledge of local community programs and resources.
  • Cultural competency and sensitivity to diverse immigrant communities.

Education and Experience

  • Associate degree or higher in Medical, Social Science, or related field (other relevant backgrounds may be considered).
  • Certification as a Community Health Worker or Medical Interpreter preferred.
  • Prior experience working with immigrant or underserved populations is highly desirable.
