Communicate the mission and vision of Creative Consulting Services, Inc. to individuals, families, and community resources.

  • Ensure individual choice is fully offered and acknowledged while maintaining a focus on safety, health, and quality of life.
  • Collaborate with families, natural supports, service providers, physicians, and other community resources.
  • Advocate for the needs of individuals as identified.
  • Complete all required documentation accurately and on time. This includes, but is not limited to:
  • Documenting all consumer and consumer-related contacts
  • Reporting serious or unusual incidents
  • Completing DMA-7, ISP, and SIS documentation
  • Facilitate and assist in obtaining and maintaining Medicaid eligibility for individuals served.
  • Facilitate ISP meetings effectively.
  • Maintain a caseload of up to 40 individuals.
  • Support the development of community-based natural supports for individuals.
  • Participate in all required training and in-service activities.
  • Monitor and respond to assigned voicemail and email regularly, ensuring responses within 24 hours.
  • Maintain a professional demeanor and dress appropriately when representing Creative Consulting Services, Inc.
  • Stay current on all state standards, guidelines, and policies related to service provision for individuals with developmental disabilities, as outlined in the CCS Policy and Procedure Manual.
  • Demonstrate proficiency in technology and software relevant to the role, including but not limited to:
  • Basic internet access and navigation

Perform additional duties as assigned by the Area Coordinator and Administrative Staff in response to contractual updates or requirements.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology , or a closely related field. Required

Minimum of one year of experience working with individuals with developmental or intellectual disabilities.
