in every interaction Position Summary The Registered Nurse RN provides nursing care to the terminally ill Hospice patient as needed To aid and understanding to the family in the home care situation and in time of bereavement and to work as a member of the Hospice team in providing Hospice care Required Experience for Registered Nurse Graduate from an accredited school of nursingCurrently licensed as a registered nurse to practice in the stateMinimum 2 years of experience as a registered nurse preferredActive patient contact within past three years preferredAbility to work independently make accurate and at times quick judgments Ability to supervise others appropriately Ability to respond appropriately to crisis outside of a hospital setting Acceptance of and adaptability to different social racial cultural and religious modes Must Complete Hospice training programAccountability for Registered Nurse Regularly assesses and reassesses the nursing needs of the Hospice patientsProvides dietary counselingProvides Hospice nursing services treatments and preventive proceduresInitiates nursing procedures appropriate for the patients Hospice care and safetyObserves signs and symptoms and reports to the physician and IDG members any unexpected changes in the patients physical or emotional conditionTeaches supervises and counsels the Hospice patient and family members about providing care for the patientSupervises and trains other nursing service personnelDevelops and re evaluates the patientfamily care plan in conjunction with IDG to meet needs and maintain continuity of carePerforms specific nursing procedures as needed eg treatments management of symptoms following doctors ordersAttends team conferencesMaintains records as required by HospiceFollows the policies and procedures of Hospice Observes confidentiality and safeguards all patient related information in compliance with HIPAA regulationsAlways communicates to the supervisor if unable to meet a patients need or perform a procedureParticipates in on call system and is responsible for providing on call coverage when unavailable for assigned dutiesWorks with interdisciplinary group concept of patient careCoordinates the implementation of the plan of care for patients residing in SNF NF ICF or MROrganizes work schedule and utilizes time management to be able to attend all required meetingsComplies with agency infection control policies and protocolsAssist with orientation teaching and training as requestedOther duties as assigned by DirectorGeneral time While performing the duties of this job there will be considerable physical effort most of the day including kneeling squatting reaching twisting climbing walking exposure to temperature and humidity changes and maximal assist in lifting andor transferring of a 100 pound patient Must possess sighthearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position Must have a current valid drivers license auto liability insurance and reliable transportation as travel may be required The work environment requires appropriate interaction with others The noise level in the work environment is moderate Occasional travel to different locations may be required This classification description is
