We’re seeking a Board-Certified or Board-Eligible Geriatrician (MD/DO) with a passion for improving cognitive and behavioral health outcomes for aging adults. In this field-based role, you’ll support dementia care navigation through a combination of in-person visits (clinic, home, and senior living communities) and telehealth, while also playing a key role in shaping the program’s clinical development.

What You’ll Do:
As a Geriatrician with Brigade Health, you will:

Conduct annual cognitive assessments and comprehensive geriatric evaluations

Deliver dementia-focused care across clinic, home, and telehealth settings

Manage chronic comorbidities and behavioral health symptoms associated with cognitive decline
-
Lead post-discharge follow-ups to reduce hospital readmissions and support transitions of care

Educate families and facilitate advanced care planning with compassion and clarity

Collaborate with our interdisciplinary team to build and refine dementia care protocols and workflows
You Are:

A licensed MD or DO , Board Certified or Board Eligible in Geriatrics, Internal Medicine, Family Medicine, or Psychiatry (Geriatric subspecialty preferred)

Experienced in or passionate about dementia care, geriatrics, behavioral health, or complex chronic care

Skilled in providing care across multiple settings with strong communication and diagnostic abilities

Mission-driven and eager to contribute to program innovation and senior care transformation
