Job Summary This position is in the Department of Aging and Community Living (DACL), Community Transition Team. The mission of DACL is to advocate, plan, implement, and monitor programs in health, education, employment, and social services which promote longevity, independence, dignity, and choice for the city’s older adults (ages 60 years or older), people living with disabilities (ages 18-59 years), and caregivers.

Duties And Responsibilities Provides detailed messaging about community transition and related home and community-based services to nursing home professionals, community-based organizations, District residents, family members, caregivers, and other District agencies. Generates and applies new concepts (Person Centered approach) in program messaging, marketing and advertising materials, and when addressing the public. Works with ADRC’s Community Transition Team and other stakeholders to ensure person-centered planning, which would include advocates, government officials, non-profit-and-for-profit interest groups and organizations addressing District adult residents with a disability, and residents ages 60+ seeking to transition into the community seamlessly.

Conducts monthly community outreach training and builds collaborative relationships with key representatives of DC Medicaid supported licensed nursing home facility in the DMV area. Develops working relationships with nursing home residents, caregivers, family members, legal guardians, long-term care advocates, community case management and home health agencies to ensure understanding of community transitions services.

Qualifications And Education Specialized Experience: Specialized experience must have equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position to be filled. To be creditable, applicants must have one (1) year of specialized experience equivalent to at least the next lower grade level in the line of progression.

An example of Specialized Experience is: working with a senior or person with a disability in their home, community or hospital, long-term care or nursing home setting to obtain services; working with clients and their care givers evaluating and coordinating social service options; working with clients in long-term care facilities to address individual transitioning needs, professional advocate and legal requests, and to address basic community responses during the nursing home discharge planning process; or similar work experience.

Working Conditions/Environment The work environment is both office and community settings. It requires frequent local travel to various nursing facilities, long-term care and assisted living facilities, off-site events, training, and other community-based settings.

Other Significant Facts Tour of Duty: Monday-Friday; Time Schedule Varies (i.e. 8:00 a.m. — 5:30 p.m.)

Position Designation: The incumbent of this position will be subject to enhanced suitability screening pursuant to Section 411 of Chapter 4 of DC Personnel Regulations, Suitability — Protection Sensitive.

Emergency Designation: This position has been designated as Emergency. Employees occupying positions designated as Emergency are required to provide advice, recommendations, and/or specific functional support necessary for the continuity of operations during a declared emergency. Report to their duty station, when activated, on time and as scheduled when a situation or condition occurs during non-work hours, and results in the late arrival or closing of District government offices for non-essential/non-emergency employees.
