Description Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN. We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life.Schedule Available: Monday – Friday.   • Provide skilled nursing care
  • Provide case management services
  • Conduct initial assessments of new patients
  • Travel to patients wherever they reside: Home, nursing facility etc.

Requirements

  • Active Registered Nurse (RN) License in the state of Indiana.
  • At least 1 year of RN experience, preferably in Hospice or Home Health.
  • Drivers License in good standing.
