Registered Nurse Hospice Case Manager – Field Position

Responsibilities: • Visit patients at home to assess needs and manage symptoms • Create and adjust care plans that fit each patient’s situation • Provide hands-on nursing care that focuses on comfort, dignity, and quality of life • Work closely with our medical director, social workers, and hospice aides to ensure seamless care • Support and guide families through the hospice journey with compassion and clarity • Advocate for each patient’s wishes, values, and cultural traditions • Take part in team meetings to share updates and coordinate care • Teach families what to expect and how they can best help their loved one • Keep thorough, timely documentation to support quality care and compliance Qualifications: • Must be willing to drive • Active Registered Nurse (RN) license in New Jersey • 2–3 years of clinical nursing experience (hospice, palliative care, or related field preferred) • Knowledge of hospice philosophy and end-of-life care • Strong communication and interpersonal skills for working with patients, families, and team members • Ability to work independently and as part of an interdisciplinary team • Excellent time management, organization, and attention to detail • Comfortable using electronic health records (EHR) and related healthcare software About Company: Spero Hospice is a group of dedicated, hardworking professionals who understand that individuals and families entering hospice care may feel scared, overwhelmed, sad, or unsure.
