Are you passionate about making a difference in others lives Then come join the Straight and Narrow team Headquartered in Paterson NJ Straight and Narrow is the largest program of its kind We provide innovative and compassionate services to individuals struggling with substance use and co occurring disorders We are looking for dedicated caring and compassionate people to work in our ResidentialOutpatient Treatment Facilities and Community Programs Lead Case Manager Residential Program Location All Straight & Narrow Locations Regular Salary 2300 2500Hr Schedule per week Full Time Monday Friday 830am 430pm Job Summary The Lead Case Manager functions as an integral part of the treatment team and provides primary social service care to a caseload of residential addiction treatment clients in residence Responsibilities Provide case management for a caseload of residential addiction treatment male or female in residence including the following tasksServe as the Lead Trainer for new case managers clinical and OP residential life staff and assist in cross training Serve as a liaison between Straight & Narrow Drug CourtProbation DCP&P and all other social service agencies including the scheduling of Case Conferences onsite visits as necessaryAscertain whether client needs birth certificates social security cards or an identification card Determine status of their TANF food stamps Medicaid benefits if applicableSend for ador make appointments for client to go for any of the above that are missingOpen clients welfare case in Passaic County andor county of origin when completing treatment Obtain any documents necessary to do this Close clients welfare case in sending county if applicableOpen clients Medicaid case immediately upon receipt of all required documents if applicableAssist in training new case managers in all departmentsSet up a Case Conference with clients DCP&Preferral source within first month of clients admission Schedule outside appointments for welfare WIC social security Probation visits Section 8 applications job search trips to IOP for interviews etcClose out clients account at S & N before he leaves transferring all personal documents to herhim Fill out form that gives client a checklist of what to do upon discharge and give her a letter to take to her first welfare appointmentClose out clients Passaic County welfare case if applicable Provide clients on caseload with referrals to other agencies ie health Psychiatric vocational rehabilitation and public assistance for concurrent and continued care as part of case managementFacilitate group and didactics as scheduled by supervisor ie Parenting Classes court orders etcHelp Coordinate childinfant donationsAssist clients with linkages to Parental Early Intervention ServicesSubmit documentation of all groups didactics and individual client sessions to supervisor dailyParticipate in community outreach services Assist with coverage of clinical responsibilities as assigned by your supervisorMaintain social services portion of clients charts as per the Division of Mental Health and Addiction Services DMHAS and CARF guidelines ensuring that the file is up to date for all documentationAttend regularly scheduled case Management Meetings MDT meetings for clients on caseload providing information on each clients progress legal situation social service benefits and housing situation Provide referrals for clients to other agencies ie health psychiatric vocational rehabilitation and public assistance for concurrent and continued social services and careEncourage clients attendance and participation in AANAGA Provide crisis intervention as necessaryFollow all Electronic Health Records EHR policiesProvide documentation of progress made towards obtaining the CADCLCADC certificationMaintain professionalism ethical conduct and appropriate boundaries at all times while delivering direct services to clients or representing the agency in accordance with agency standards and substance abuse best practices Other duties as assigned by the administration Benefits For full time positions we offer medical vision dental life and supplemental life insurance Accidental and Critical Illness Insurance 403B with employer match Paid Time off PTO in house training seminars two training days for certified professionals SN also offers supervision for those individuals trying to become licensed in NJ as a LCADCCADC LSW LCSW LAC LPC and LMFT We participate in employee discount programs such as the Working Advantage program Qualifications Must have a minimum of Highschool Diploma preferred BABS or MAMSMSW degree Minimum experience of 3 years in the human services field Enrollment in CADCLCADC courses experience in case management and knowledge of addiction and co occurring preferred Must be computer literate and possess ability to type and have a valid Drivers license
