The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient centered hospice that maximizes resources at bedside while being operated locally Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve When end of life care is necessary patients and their families are in their most vulnerable time Our Ohio based management team through our local clinicians can respond to the needs of our patients in a timely and appropriate manner because we are local We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton OH area We dont require high caseloads or unmanageable quotas which gives our team more time with each of their patients Our top priority is our patients and the people serving them Schedule Monday Friday 8a 5p with two holidays on call each year What We offer Great culture and team atmosphereComprehensive benefits including medical dental and vision effective on the first of the month401k retirement plan with a generous company matchGenerous time off accrualsMileage Reimbursement Paid holidaysTuition ReimbursementEmployee Referral ProgramMerit IncreasesEmployee Discount ProgramsWhat Youll Do Provides direct patient care as defined in State Nurse Practice ActImplements current nursing practice following a comprehensive assessment and the plan of careProvide accurate and timely documentation of patient services to reflect the plan of careAssess and provide patient and familycaregiver education and information pertinent to diagnosis and self plan of careParticipates in coordination of hospice services appropriately reporting the identified needs to the interdisciplinary group Examples include hospice aide OT PT MSW Dietitian or Clinical SupervisorProvides appropriate painsymptom management Evaluates patients response to treatmentsmedicationsQualifications Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state Prior home health or hospice experience preferred w minimum of 1 year medicalsurgical experience Maintains a current CPR certification Must be a licensed driver with an automobile that is insured in accordance with state andor organization requirements and is in good working order
