Come work for one of the largest Home Health Care companies in the State of New Mexico Ambercare Homecare is looking for Registered Nurse to come join our rapidly growing team in Alamogordo NM PRN We offer Great culture and team atmosphere401k retirement plan with a generous company matchMileage reimbursementEmployee Referral Program Merit IncreasesEmployee Discount ProgramsWorklife balanceWhat Youll Do Completes an initial assessment of patient and family to determine home care needs Provides a complete physical assessment and history of current and previous illnessesObserve and report any signs or symptoms indicative of changes in condition of the patient and family situation Document such reports and observations in the clinical record and notify supervisorRegularly re evaluates patient nursing needsInitiates the plan of care and makes necessary revisions as patient status and needs changeUses health assessment data to determine nursing diagnosisDevelops a care plan which establishes goals based on nursing diagnosis and incorporates therapeutic preventive and rehabilitative nursing actions Includes the patient and the family in the planning processInitiates appropriate preventive and rehabilitative nursing procedures Administers medications and treatments as prescribed by the providerProvides direct patient care as defined in the State Nurse Practice ActCounsels the patient and family in meeting nursing and related needsProvides health care instructions to the patient as appropriate per assessment and plan of careIdentifies discharge planning needs as part of the care plan development and implements prior to discharge of the patientActs as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseloadPrepares clinical notes and updates the primary provider when necessary and at least every 60 daysCommunicates with all relevant providers regarding the patients needs and reports any changes in the patients condition; obtainsreceives orders as requiredCommunicates with community health related persons to coordinate the care planPerforms other duties as assigned Qualifications Graduate from an accredited registered nursing program and licensed in the stateAt least one 1 year clinical practical nursing experience preferably in intensive care rehabilitation or medical surgical nursing preferredHome health experience preferredPossess and maintain valid CPR certificationMust have reliable transportation current drivers license and appropriate automobile insuranceText 10009 to 505 806 1865 to apply ACHH
