Capital City Hospice is proud to be Ohio made Every decision we make about hospice care is made with patients and families in mind Our team is rooted in the communities we serve and are dedicated to delivering the highest quality of care for our fellow Central Ohio residents We believe that the best definition of care is determined by the wishes and needs of patients and their families We are committed to supporting our patients to remain wherever they call home throughout their entire journey To accomplish this we strive to support not only our patients but the team that serves them Our team carries low patient caseloads and are never on call We provide a positive work experience for our employees by empowering our staff and giving them rewarding careers with manageable workloads support and flexibility Come join the fastest growing hospice care in the Midwest Schedule Monday Friday 8a 5p with 1 2 weeks of admin on call per month What We offer Great culture and team atmosphereComprehensive benefits medical dental vision lifeAD&D disability effective on the first of the month401k retirement plan with a generous company matchGenerous time off accrualsPaid holidaysMileage reimbursementTuition ReimbursementEmployee Referral Program Bonus EligibleHospice Certification benefit and salary increaseMerit IncreasesEmployee Discount ProgramsWhat Youll Do Ensures that care is provided in accordance with Medicare Medicaid Federal and state guidelines Ensures that all direct care clinical staff are assigned appropriately Monitors business operations to insure financial stability Monitors operational progress toward accomplishing operational and strategic goals Establishes and maintains effective open channels of communication Ensures adequate and appropriate staffing and staff supervision Ensures complete appropriate and adequate staff development including orientation in service continuing education competency testing and quality assessment performance improvement Responsible for the management of the hospice program in matters of clinical operation Demonstrates exceptional service to patients families referral sources co workers and other customers Assists team members as needed to ensure that this level of service is consistently provided Active participant in receiving case referrals Reviews available patient information related to case including home visits to determine hospicehome care needs Assigns appropriate hospice personnel to case as needed Conferences with Medical Director regarding any questions about an individuals eligibility for servicesEnsures that Interdisciplinary Group IDG meetings are regularly conducted and records maintained Attends IDG meetings to facilitate coordination of care and discussion of interdisciplinary group involvementQualifications Graduate from an accredited registered nursing program BSN preferred One year hospice experience required Two years healthcare management experience required Must have good knowledge of hospice Medicare Medicaid State and Federal guidelines Maintains a current CPR certification Must be a licensed driver with an automobile that is insured in accordance with state andor organization requirements and is in good working order
