Ambercare is looking for a full time LMSWLCSW to join our fast growing hospice team in the city of Deming Earn up to 70000 annually immediate openings mileage reimbursement and 401k match What We offer Great culture and team atmosphereComprehensive benefits medical dental vision lifeAD&D disability401k retirement plan with a generous company matchGenerous time off accrualsMileage ReimbursementPaid holidaysTuition ReimbursementEmployee Referral ProgramMerit IncreasesEmployee Discount ProgramsWhat Youll Do Performs the psychosocial assessment of patients and familiescaregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patients plan of care Provides an assessment in the patients identified residence and assistance when this is not safe and another plan is requiredDemonstrates exceptional service to patients families referral sources co workers and other customers Assist team members as needed to ensure that this level of service is consistently providedCounsels patient and family caregivers and staff as needed in relationship to stress and other identified coping difficultiesProvides accurate and timely documentation of patient services to reflect the plan of careProvides assistance with applications for Supplemental Security Income Hospice Medicaid and Medicaid Waiver Obtains resources from community social health pharmaceutical and welfare agencies to assist in meeting patientfamily needsServes as liaison between patients and familiescaregivers and community agenciesQualifications Social Work DegreeNew Mexico Masters Degree in Social Work requiredCurrent State Licensure in Social WorkNew Mexico LMSW or LCSWMinimum of one years experience in health care or hospice settingValid drivers license and proof of insurance is required
