This position is eligible for a 2500 retention bonus Job Summary The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms Duties and Responsibilities Coordinate a variety of services Provide supportive counseling problem solving and limit setting Participate as a member of a multi disciplinary team in the development and implementation of therapeutic services to include working with people who have sever persistent mental illness andor substance abuse issues as it relates to case management Monitor client status during spend down process complete necessary DHS applications In cooperation with Managers or Emergency Services Staff assist with assessments crisis interventions and risk management as needed Negotiate and advocate on behalf of the consumer as needed with landlords courts families the treatment team and consumers themselves Transport consumers in own vehicle to planned activities and appointments Complete recordkeeping in accordance with Health Information Services HIS and other TPC requirements within required time frames and abide by TPC confidentiality policy Maintain cooperative relationships with Center staff consumers community agencies and the public Attend mandatory in service trainings and other trainings required Attend case presentations and conferences Requirements Associates degree required Bachelors degree from accredited four year College or University in Human Services or related field preferred Experience in Human Services field preferred Bilingual Spanish candidates preferred Must have valid drivers license and insured auto Must be able to work flexible hours Care New England Health System CNE and its member institutions; Butler Hospital Women & Infants Hospital Kent Hospital VNA of Care New England Integra The Providence Center and Care New England Medical Group and our Wellness Center are trusted organizations fueling the latest advances in medical research attracting top specialty trained doctors and honing renowned services and innovative programs to engage in the important discussions people need to have about their health Americans with Disability Act Statement External and internal applicants as well as position incumbents who become disabled must be able to perform the essential job specific functions either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case by case basis EEOC Statement Care New England is an equal opportunity employer All applicants will be considered for employment without attention to race color religion sex sexual orientation gender identity national origin veteran or disability status Ethics Statement Employee conducts himselfherself consistent with the ethical standards of the organization including but not limited to hospital policy mission vision and values
