Residential Case Manager Brick Academy $1,000 retention bonus
This position is eligible for a prorated 1000 retention bonus Job Summary The Providence Center Residential Case Manager Brick Academy provide clinical case management services in order to facilitate program goals and objectives using a quality care approach Identify client needs with a person centered approach to address not only mental health symptoms but co occurring disorders Working with consumers to achieve the highest level of independence possible Duties and Responsibilities Initiate treatment and advocate for consumer within network of care Assist in the development and implementation of socialization and recreational activities within the group home in the community and by attending Day Program groups and activities Complete recordkeeping in accordance with Health Information Services HIS and other Center requirements and abide by Center confidentiality policy Provide case management and support to consumers Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments Initiate treatment and advocate for consumer within network of care Assist in the development and implementation of socialization and recreational activities within the group home in the community and by attending Day Program groups and activities Transport consumers in own vehicle or TPC van if available to planned activities and appointments Complete recordkeeping in accordance with Health Information Services HIS and other Center requirements and abide by Center confidentiality policy Maintain cooperative relationships with Center staff consumers community agencies and the public Other duties as assigned Requirements High School DiplomaGED required; Associates or Bachelors degree preferred If no higher education degree must have 5 years of relevant behavioral health experience Experience in residential facilities is preferred Bilingual Spanish preferred Must have valid drivers license registration current inspection and reliable insured automobile with adequate liability coverage Care New England Health System CNE and its member institutions Butler Hospital Women & Infants Hospital Kent Hospital VNA of Care New England Integra The Providence Center and Care New England Medical Group and our Wellness Center are trusted organizations fueling the latest advances in medical research attracting top specialty trained doctors and honing renowned services and innovative programs to engage in the important discussions people need to have about their health EEOC Statement Care New England is an equal opportunity employer All applicants will be considered for employment without attention to race color religion sex sexual orientation gender identity national origin veteran or disability status Ethics Statement Employee conducts himselfherself consistent with the ethical standards of the organization including but not limited to hospital policy mission vision and values Americans with Disability Act Statement External and internal applicants as well as position incumbents who become disabled must be able to perform the essential job specific functions either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case by case basis
