Providence Hospice Oregon is seeking a full-time, certified nurse practitioner in Adult, Family, Acute or Geriatric Medicine to join a skilled interdisciplinary team providing coordinated, comprehensive Hospice care to patients and families in the Portland, Oregon metro area. The nurse practitioner in this role will be providing emergency assessments, recertifications, symptom management, educational and evaluative services, assists patients and families with goal setting related to advanced care planning and end-of-life process. Ideal candidates will be advanced Hospice and Palliative certification or willing to pursue and have a strong interest in end-of-life care. Providence Hospice Oregon is part of a growing integrated Home and Community Services program which collaborates with Home Health, Palliative Medicine, Geriatrics as well as hospital-based practitioners to care for patients and their families.

  • Full-time

  • Monday to Friday, daytime hours

  • Outpatient, community-based setting; may do virtual visits occasionally

  • Call 1 weekday per week and 1 weekend per month

  • Ideal candidates will have advanced Hospice and Palliative certification or willing to pursue and have a strong interest in end-of-life care

Where You’ll Work

The Oregon Region Hospice Program is responsible for the medical direction and oversight of hospice services throughout the region’s ministries, including care provided in home settings and intermediate care, assisted living and acute care facilities. The primary goal is to provide support that allows patients to spend their time living as they wish, ideally in familiar surroundings in the company of family and friends.
