Description
We’re seeking an Transfer Center Registered Nurse is responsible for accepting incoming referral information via multimedia and working directly with bed management and direct care nursing to coordinate inpatient bed assignments (particularly critically ill) for patients at Emory Healthcare. This indvidual:

Coordinates, prioritizes, and optimizes patient flow using knowledge of clinical and operational activities throughout the hospital network.

Collects clinical information to determine patient’s level of care while collaborating with providers to arrange acceptance by the attending EHC physician to the most appropriate EHC hospital.

Monitors bed availability and coordinate bed assignments for all patients receiving care in an inpatient or outpatient bed and for patients being transferred between units and from outside facilities to maximize bed utilization.

Communicates with referring hospital to coordinate transportation arrangements and provide an assigned bed.

Communicates clinical information to transportation company, physicians, physician designees, referring facilities, and charge nurses.

Ensures safe and timely placement of patients based on admission status, physician requested level of care, patient’s clinical information and nursing units admission/discharge criteria.

Maintains the patient transfer and flow software to accurately reflect physical location of all patients occupying an inpatient bed and/or ancillary holding area.

Confirms patient insurance eligibility and obtain specific payor benefits.

Obtains and confirms admission notifications and authorizations for external clinic/facility transfers.

Documents findings in the Financial Billing System Exhibit appropriate customer/guest relations in all interactions.

Consistently communicates information in a timely, accurate and concise manner.

Apprises Manager of significant developments affecting patient flow.

Participates on Nursing and Hospital committees and councils as required.

Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
-
Bachelors degree in nursing required. Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.

Experience: Three years of recent (within the past five years) critical care, procedural or emergency room nursing experience that includes supervision and patient bed placement highly preferred. Any combination of direct nursing patient care experience may be considered.

Knowledge, skills, and abilities: Strong critical thinking, accurate assessment and problem solving, excellent verbal and written communication skills, attentiveness to detail, high-level analytical ability required in order to resolve complex patient flow issues, strong interpersonal skills, knowledge in nursing concepts, practices and procedures.
