Compensation and Benefits:
Base Salary: $100,000 — $150,000

Position Overview
Working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of residents within skilled nursing facilities. The APRN will build strong working relationships with administrators and staff throughout their designated building and serve as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what they love—working independently to see patients and treat complex medical conditions.

Nurse Practitioner Job Description
This is a full-time position, working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and staff throughout their designated building(s) and will act as the primary point of contact for emergencies and urgent care visits. What Does a Day Look Like for a UHP APRN?
You will provide primary, urgent and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents
You will verify admission orders and conduct initial assessment of residents. Conduct physical exams, medication review, and depression screening
You will collaborate with facility Medical Director, attending physicians and nursing staff to deliver the highest level of care to the patient
You will communicate with and counsel family members on treatment options when there are material changes in status

Responsibilities

Minimum Qualifications


  • Valid Nurse Practitioner License in applicable state
  • DEA License
  • Nurse Practitioner national certification
  • Clinical background in adult, family, or geriatric specialties
  • COVID-19 vaccination required unless qualified for a remote, religious, or medical exemption

Preferred Qualifications


  • Experience in internal medicine, gerontology, or cardiology
  • Excellent administrative, organizational, and communication skills
  • Ability to work independently
  • Detail-oriented, dependable, and reliable
  • Nursing experience in LTC/SNF a plus
