  • Salary range: $100,000 — $115,000 annually
  • 25 PTO days in year one
  • 6 paid holidays

Responsibilities

  • Setting: Home Health

  • Patient volume: 6–7 per day

  • Productivity: 33 points/week

  • Admission — 2 pts

  • Recert/Resumption — 1.25 pts

  • Routine Visit — 1 pt

  • Infusion Admission — 2.5 pts

  • Infusion Revisit — 1.5 pts

  • Schedule: Mon–Fri, 9am–5pm

  • Weekends: 2 weekend days/month (fee-for-service)

  • No on-call duties

  • EMR: Homecare Homebase (HCHB)

  • Documentation: OASIS preferred

Requirements

  • Active RN license in NY
  • Associate or Bachelor’s degree in Nursing
  • 1+ year clinical nursing experience preferred
  • OASIS experience strongly preferred
  • Home health experience a plus
  • Reliable transportation required
