  • Salary range: $100,000 — $115,000 annually
  • 25 PTO days in year one
  • 6 paid holidays

  • Fee-for-service rates offered on weekend visits

Responsibilities

  • Setting: Home Health

  • Patient volume: 6–7 patients per day

  • 33 productivity points required weekly

  • Admission — 2 points

  • Recertification/Resumption of Care — 1.25 points

  • Routine Visit — 1 point

  • Infusion Admission — 2.5 points

  • Infusion Revisit — 1.5 points

  • Schedule: Monday through Friday, 9:00am to 5:00pm

  • Weekend requirement: Two weekend days per month (paid fee-for-service)

  • No on-call requirements

  • EMR: Homecare Homebase (HCHB)

  • Documentation: OASIS experience preferred

Requirements

  • Active Registered Nurse (RN) license in the state of New York
  • Associate or Bachelor’s degree in Nursing
  • Minimum of one year of clinical nursing experience preferred
  • OASIS experience strongly preferred
  • Previous home health experience is a plus
