• Provide expert clinical pharmacy services, including medication therapy management for chemotherapy patients
• Collaborate with healthcare providers to develop and adjust medication plans ensuring optimal patient care
• Maintain accurate documentation of patient medications and interventions in the hospital’s EMR system
• Educate patients and staff on chemotherapy drugs, side effects, and safety protocols
