- Education Requirements: Bachelors degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited institution
- Certifications Requirements: Active pharmacist license in Oregon; Basic Life Support (BLS); Pharmacy Compounding Certification (preferred)
- Experience Requirements: At least 1 year of clinical pharmacy experience; experience with chemotherapy drugs; familiarity with hospital pharmacy protocols

- Provide expert clinical pharmacy services, including medication therapy management for chemotherapy patients
- Collaborate with healthcare providers to develop and adjust medication plans ensuring optimal patient care
- Maintain accurate documentation of patient medications and interventions in the hospitals EMR system
- Educate patients and staff on chemotherapy drugs, side effects, and safety protocols
- Execute medication preparation and dispensing in accordance with hospital policies and safety standards
