Responsibilities of a Senior UAS Assessor:

• Join a mission-driven care management team performing UAS Assessments and supporting long-term care members through personalized care plans, hands-on coordination, and meaningful advocacy to ensure they receive the highest quality care across the continuum.

Required Qualifications of a Senior UAS Assessor:

• Active and clear NY State RN license
• Experience in UAS, hospital, or home health settings
