Job Summary: To support individuals in attainingthe highest quality of life while maintainingtheir safety across all settings.Supervisory Responsibilities: NoneEssentialDuties/Responsibilities:Implement Positive Behavior Supports (PBS)Report any significant issues using the on-call hierarchyImplements and follows individuals’ Behavior Skills PlansImplements and follows EO509 Dining GuidelinesParticipated in and completed Licensing and Certification paperwork which includes but notlimited to Daily Planners. Monthly Menus, House Meetings, Budgeting, Medical Appointments, etc.Follows and Implements individuals’ daily schedulesEnhance an individual’s development by providing job coachingMaintain appropriateand professional boundaries with the individualsInvolving individuals in social and vocational opportunitiesImplementing Individual Service Plan (ISP) objectivesPromote a high quality of living for the individuals which includes but notlimited to assistingwith showers, toileting, brushing teeth, laundry, etc.Able to support and accompany individuals in the community including but not limited to religious services, hospitals, correctional facilities, courts, support groups, etc.Record accurateand quality data on each individualthroughout the shift using ICentrixsProvides supervision of the individuals and ensures their safety in the residence and communityPromote a high quality of living that includes routine maintenance/housekeeping which includes but not limited to shoveling, mopping, cleaning appliance, etc.Provide Transportation; whether in the company vehicle or your personal vehicle, for individuals including following the GPS GuidelinesPerform verbal or physical interventions as needed to ensure safetyConduct physical restraints as needed.Must be able to maintainHIPAA, Confidentialityand Cyber Security.Extended standing, sitting, runningand lifting as needed to ensure safetyRequiredSkills/Abilities:Strong verbal and written communication skillsAble to accept constructive criticism as a learning toolAble to handle intimate and personal issues with tact and discretionAble to work in a team settingAble to stay awake and alert while on shift, at all timesFlexibility: May be requiredto work at different locationsand shifts to accommodate business needs by arriving to shift on time, maintainingcontracted ratios, working holiday shifts, and weekendsAble to calmly and patiently manage threatening and assaultive situationsAble to act appropriately in the event ofan emergencyBasic computer skills (email, databases, internet, etc) copy, scanand faxRequired Education and Experience: High school diploma or equivalent.Valid Driver’s LicensePABC (physical restraints)CPR/First AidMAPDDS Licensing and Certification required trainingPreferredEducation and Experience:1 year of residential experienceTravel requirements: Heavy Travel – once or more a weekOther duties Please notethis job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
