• Provide emotional support, counseling, and guidance to residents/patients and their families to address psychosocial needs and concerns under the direction of a Masters Level Social Worker.
• Assist in assessing residents’/patients’ social and emotional needs and developing individualized care plans in collaboration with the interdisciplinary team.
• Maintain accurate and up-to-date documentation of social work assessments, interventions, and progress notes in accordance with regulatory standards.
• Assist residents/patients and their families in accessing community resources, support services, and financial assistance programs as needed.
• Advocate for residents’/patients’ rights and interests and collaborate with healthcare teams to ensure their psychosocial needs are addressed effectively.
• Provide education and support to families on coping strategies, communication skills, and caregiving responsibilities.
• Assist in managing crisis situations and providing emotional support and crisis intervention services to residents/patients and families during challenging times.
• Collaborate with healthcare professionals, including nurses, physicians, therapists, and administrators, to coordinate care and address psychosocial issues impacting residents’/patients’ well-being.
• Participate in ongoing training and professional development opportunities to enhance knowledge and skills in social work practice and geriatric care.

Assistant Social Worker Qualifications:

• Bachelors Degree in social work or related field.
• Previous experience in a social work or related role preferred but not required.
• Strong communication and interpersonal skills, with the ability to build rapport and establish trust with residents/patients and families.
• Compassion, empathy, and a patient-centered approach to care.
• Knowledge of social work principles, ethics, and practices.
• Ability to work independently and as part of a team in a fast-paced environment.
• Proficiency in computer applications and electronic health records systems..
• Commitment to maintaining confidentiality and adhering to professional ethics and standards.
