Position offers aggressive compensation, benefits, paid time off and more!Job Summary: The Physical Therapist provides skilled physical therapy services to patients in their homes, helping them regain mobility, improve strength, and increase independence following illness, injury, surgery, or hospitalization. This role involves evaluating patient needs, developing personalized care plans, and working closely with interdisciplinary healthcare teams to deliver high-quality, patient-centered care in a home environment.Key Responsibilities: Perform initial and ongoing physical therapy evaluations in the patient’s home.Develop and implement individualized treatment plans based on clinical findings and physician orders.Provide hands-on physical therapy interventions including therapeutic exercises, gait training, balance work, and functional mobility training.Educate patients and caregivers on safety, assistive devices, home exercise programs, and strategies for fall prevention.Reassess patients regularly and modify care plans to reflect progress or changes in condition.Accurately document visits, assessments, goals, progress, and outcomes in the agency’s electronic medical record (EMR) system.Communicate and coordinate care with physicians, nurses, occupational therapists, and other members of the care team.Participate in discharge planning and make recommendations for durable medical equipment (DME) or further services as needed.Adhere to all applicable federal, state, and agency regulations (including HIPAA, Medicare guidelines, and infection control protocols).Requirements: Doctor of Physical Therapy (DPT) or Master’s degree from an accredited Physical Therapy program.Current state license to practice Physical Therapy.Excellent communication, organization, and documentation abilities.Valid driver’s license, reliable transportation, and auto insurance (travel required).Prior experience in home health or geriatrics preferred.Benefits include:
