We are seeking a Clinical Program Coordinator (NP or PA) to support the growth and quality of our Clinical Services Team. This hybrid position combines 50% leadership/supervision with 50% direct patient care, allowing you to guide programs while managing complex post-acute patients.The role includes 25-50% travel across Wisconsin and Midwest partner sites to ensure consistency, quality, and operational success.Position Overview The Clinical Program Coordinator oversees and enhances clinical programs, ensuring high-quality care and operational efficiency. Serving as the lead resource for assigned facilities, this role collaborates with providers, interdisciplinary teams, and leadership to drive clinical excellence and performance improvement.Ideal candidates are experienced NPs or PAs who enjoy blending leadership with hands-on care, thrive in dynamic settings, and are committed to quality and collaboration.Key Responsibilities Clinical Oversight (50%)Supervise, mentor, and evaluate clinical staff and studentsLead protocol implementation, performance improvement, and compliance initiativesOversee orientation and training for new providers and programsReview documentation for accuracy and regulatory adherenceSupport new program launches and business startupsPatient Care (50%)Evaluate, treat, and manage patients in collaboration with physicians and facility staffCoordinate care plans, monitor outcomes, and provide educationFacilitate discharge planning and ensure timely documentationDevelop standardized protocols and templates to maintain care qualityLeadership & CollaborationLead program development and improvement initiativesPromote accountability, teamwork, and excellenceRepresent the organization at meetings and industry eventsSupport strategic and budgetary planningQualifications Licensed or eligible NP/PA in Wisconsin5-8 years of clinical experience (hospital, post-acute, or complex care preferred)Strong leadership and communication skillsCommitment to quality improvement and patient-centered careAbility to travel 25-50%Meets DOL “medium work” physical requirementsCompensation & Benefits Competitive salaryHealth, dental, and vision coverageShort- and long-term disabilityLife insurancePaid time off (full-time employees)
