- Facilitates the patient’s continued recovery after discharge. Provides aftercare plan coordination for patients who need ongoing medical/psychosocial care following discharge. In collaboration with other members of the healthcare team, assesses medical needs of the patient and offers suggestions regarding follow-up care options.

- Bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
- Prior experience with psychiatric and chemical dependency patients.
- Unencumbered clinical license if eligible/needed per state regulations preferred. CPR certification and Crisis Prevention Training (CPI) within 30 days.
