Job Description
The Last Mile team focuses on ensuring a smooth delivery experience right to the customer’s doorstep. Within this team, we specifically handle incident reporting and investigation of deliveries made on the road. We play a crucial role in ensuring timely reporting of incidents and conducting thorough investigations to maintain compliance with mandatory reporting requirements.

About the Role:
As an Incident Reporting Investigator, you will be responsible for auditing and investigating last-mile delivery incidents to ensure compliance with mandatory 24-hour reporting policies. You will leverage established Standard Operating Procedures (SOPs) and various tools to conduct thorough investigations, determine when incidents were reported, and verify reporting channels. This role requires strong investigative skills across multiple platforms, excellent analytical abilities, and professional communication skills for Delivery Service Provider (DSP) outreach. You will also contribute to process improvement initiatives by identifying trends and root causes.

Key Job Responsibilities:
Conducting thorough audits and investigations using established SOPs to verify incident reporting compliance.
Determining when incidents were reported to client and through which channels.
Evaluating incidents against mandatory 24-hour reporting policy requirements.
Performing outreach to DSPs to collect additional evidence and information related to incidents.
Analyzing data across multiple platforms to determine root causes of incidents and reporting delays.
Working collaboratively with team members to navigate complex cases.
Ensuring accurate documentation and evidence collection throughout investigations.
Identifying and escalating trends or pain points within the process based on investigation outcomes.
Utilizing intermediate Excel skills, including pivot tables, for documenting investigation data and data analysis.

Story Behind the Need – Business Group & Key Projects

Business Group:

  • Team is the reporting incidents compliance mandatory enforcement plan
  • DFP’s are contractually required to report safety incidents on the road (illness, hospital, collisions, etc) within 24 hours – can be high severity incidents
  • Team reviews weekly output that is given that may need a second glance to see if DFP’s met reporting requirements
  • Weekly work queue to see when incident happened, when it was reported, who it was reported within the company, follow up directly with DSP’s
  • Additional deep dives depending on

Key Projects:

  • 2026 looking to add additional scope

Reason for request:

  • Backfill, had 4 contractors but contractor fell through
  • For operational capacity

Typical Day in the Role

Day to Day:

  • Conducting thorough audits and investigations using established SOPs to verify incident reporting compliance.
  • Determining when incidents were reported to us and through which channels.
  • Evaluating incidents against mandatory 24-hour reporting policy requirements.
  • Performing outreach to DSPs to collect additional evidence and information related to incidents.
  • Analyzing data across multiple platforms to determine root causes of incidents and reporting delays.
  • Working collaboratively with team members to navigate complex cases.
  • Ensuring accurate documentation and evidence collection throughout investigations.
  • Identifying and escalating trends or pain points within the process based on investigation outcomes.
  • Utilizing intermediate Excel skills, including pivot tables, for documenting investigation data and data analysis
  • Ensure DSP’s are following corrective action plans if needed
