Population Health Data Coordinator (FHC)
CHARACTERISTICS OF THE WORK
Under general direction, assists the care coordination department in obtaining necessary information to proactively manage the health and preventative services of Family Health Centers (FHC) patients.
Assists FHC’s clinic leadership and Quality Improvement (QI) teams in assembling and presenting the data necessary to guide effective change in the delivery of patient care.
EXAMPLES OF THE WORK (These statements are intended to be illustrations of the work of this class.)
Creates graphic and descriptive executive summaries to maintain monthly operational dashboard data and trend reports for internal and external distribution.
Prepares and maintains monthly provider panel data for review and distribution to FHC providers and managers.
Runs reports as needed from the electronic health records (EHR) and/or from practice analytics to identify patients needing follow-up care or preventive services.
Compiles and distributes Care Team score cards.
Runs reports as needed to prepare graphs and presentations for the Quality Improvement committee, staff, and department meetings.
Runs scheduled and ad hoc reports and distributes data as needed for review of individual Quality Improvement projects and Plan-Do-Study-Act (PDSA) cycles.
Downloads and processes external care gap reports from payers into usable reports for integration into the electronic health record (EHR).
Produces monthly statistical packet data for internal and external review.
Assists in the completion of the annual Uniform Data System (UDS) report and submission to the Health Resources and Services Administration (HRSA).
Performs Related Work As Required.MINIMUM REQUIREMENTS Bachelor’s degree in Public Health, Computer Science, Computer Information Systems, Information Technology with a concentration in Data Management and Analysis, Database Administration and Data Center Operations, Mathematics, or a closely related field.
Six months of work experience extracting, compiling, and analyzing data to identify and quantify trends and key metrics.
An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.
Physical Requirements And Working Conditions Responds to patient and Family Health Centers’ needs via the telephone, internet, fax, computer software and in person.
Special Requirements Must be available to work extended hours as necessary.
Must provide own transportation to work at other FHC locations as needed.An Equal Opportunity Employer
