Position Overview
We are seeking a highly organized Project Manager (PM) to lead the implementation and coordination of Medicaid-related services for youth involved in the juvenile justice system . This role focuses on developing policies, operational models, and care coordination strategies in compliance with federal requirements, particularly Section 5121 of the Consolidated Appropriations Act (CAA) of 2023.
The ideal candidate will have experience with Medicaid, behavioral health, juvenile justice, or public systems reform and demonstrate strong project management and stakeholder engagement skills.
Key Responsibilities
Strategic Program Oversight
- Lead the development and rollout of statewide policies and operational models for service delivery to justice-involved youth.
- Oversee the design and execution of a care delivery model supporting continuity of care 30 days pre- and post-release from juvenile facilities.
- Facilitate cross-agency collaboration with healthcare and justice partners.
Policy & Compliance
- Support the development and implementation of Medicaid policy aligned with CAA 2023 mandates.
- Assist with federal reporting and documentation, including waiver and CMS submissions.
- Ensure audit readiness and adherence to Medicaid requirements related to EPSDT and Targeted Case Management (TCM).
Care Coordination & Systems Integration
- Design integrated workflows among juvenile facilities, managed care providers, and community health services.
- Establish processes for referrals, enrollment, and follow-up before and after release.
- Promote culturally responsive, trauma-informed care models.
Data, Quality, & Evaluation
- Define key performance indicators (KPIs), dashboards, and evaluation frameworks.
- Track outcomes related to service access and continuity of care.
- Work with state systems to integrate claims, eligibility, and transition data.
Stakeholder Engagement & Technical Assistance
- Organize and facilitate statewide task forces, stakeholder meetings, and training events.
- Provide technical assistance to service providers, government partners, and care managers.
- Support leadership in federal communications and implementation strategies.
