Job Description: Overview Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.Team 1 Covers; Bayville, Bethpage, East Norwich, Jericho, Glen Cove, Glen Head, Hicksville, Levittown, Massapequa, Plainedge, Seaford, Sea Cliff, Syosset, Woodbury.

• 5x 8 hour shifts (8:30AM to 4:30PM) – Rotating Weekends

Job Details Our Home Care LPN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.

Minimum Qualifications:

• New York State Licensed Practical Nurse (LPN) license
• Minimum of one year of recent hands on experience
• Basic Life Support Certification

Salary Range USD $32.00 – USD $40.00 /Hr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
